'Hospice at Home' service
In conjunction with Somerset Palliative Care Partnership we provide a hospice at home service for adults living in Somerset, and in the last stages of their life and who would like to be cared for at home.
This service provides: - skilled and practical hands-on nursing care for patients who wish to stay in their own homes
- support and respite for those who care for the patient
- 24 hour cover, dependent on need and service availability
- choice for patients about where they are cared for
- an opportunity for patients to be discharged home from hospital or hospice for their last few days where this is an expressed wish by the patients and their families
Skilled healthcare assistants stay with the patient during periods of the day or night providing practical 'hands on' care to patients and respite care for the patient's carers. This service has reduced the neccessity for people to be admitted to hospital and enabled patients to return home. Access to this service can be made via the patient's GP, District Nurse or St. Margaret’s Community Palliative Care Nurse Specialists (CPCNS) in your locality.
