Hospital based Specialist Palliative Care Teams
Our services extend in to the counties two hospitals based in Yeovil and Taunton.
 We provide 2 pallaitaive care teams who work within each of the hospitals. The teams consist of nurses and doctors who visit patients on the wards. The teams support patients in need of specialist palliative care symptom control. The team aim to alleviate troubling symptoms like pain, nausea, vomitting, breathlessness and anxiety. Working with their NHS colleagues, they strive to help patients get to their preferred place of care and when neccessary co-ordinate their transfer to the hospice.
The teams work alongside other Consultants and specialists to ensure patients and their families are supported appropriately throughout their treatment. This service operates 5 days a week Monday to Friday from 9am to 5pm. The teams also have a responsibility to teach best practice and symptom control in palliative care to other professionals, including how to break bad news.
