You will: Provide specialist palliative nursing care, advice and information that is patient-centred, holistic and quality driven, implementing the Hospice philosophy to ensure optimum quality of life for all patients across all clinical settings; Offer advice and support to meet the physical, emotional, spiritual, psychological and social needs of families, carers and bereaved relatives; Assist in managing, operating and supporting a 24/7 service for health care professionals, patients and carers, by providing appropriate clinical and non-clinical palliative care advice and support, within the boundaries of the service; Assist in managing referrals in our central referral centre. The post-holder must have: RN1 / 2 qualification or equivalent. Ability to travel to different sites and communities within the county. Flexibility to work across a range of care settings and shifts, including evenings, weekends.
